Documents and layers: create, rename, move and remove

Process a list of operations on documents and their layers: create a document, add a layer with a value to a document, change a layer's value, move a layer to another document, and remove a layer. Layer IDs are unique across all documents. At the end, return every document sorted by ID with its layers sorted by ID. Keep a map from layer to its document so moves and removals don't scan.
